Children with serious or chronic illness can spend a lot of time in hospital. This video looks at the transition from paediatric health care to adult health care. Health professionals as well as parents of teenagers with chronic conditions talk about the transition process. Two young people share their experiences of moving from child-centred care to adult-centred care.
